Reminder for Students: Complete Your Daily COVID-19 Health Assessment

If you are a student coming to campus, please complete the COVID-19 Self-Assessment. As part of the university's efforts to maintain the health and safety of our campus community, students are strongly encouraged to complete this health assessment each day they are coming to campus.

Planning to Come to Campus
You will be asked to provide your UARK username (UARK email without @uark.edu) and password to login to the COVID-19 Self-Assessment first time you take the self-assessment, which asks about potential symptoms and gives guidance if you do have symptoms.

Not Coming to Campus
Students do not need to take this assessment on a day when you are not coming to campus.

Visit the university's COVID-19 information site for more on COVID Testing, Reporting, Tracing and Quarantine procedures. If you were tested off-campus, received a positive result and have either been on campus or plan to be this semester, you must self-report using this online self-reporting form. This will alert U of A Emergency Management to initiate the contact tracing process.
