Forms for Conflict of Interest/Commitment and Outside Employment Approval Requests to Be Online
Starting Sept. 1, online forms will be available for Conflict of Interest/Commitment and Outside Employment Approval at rsic.uark.edu/other/coi. Paper forms should no longer be used.
All U of A employees should complete the Disclosure and Management of Potential Conflicts of Interest/and/or Commitment form, per Fayetteville Policy and Procedure 404.0.
The online form should be completed by all faculty and staff (new, part-time and full-time) annually and any time a new potential conflict of interest emerges throughout the year.
All faculty and non-classified staff who receive compensation outside of the university should complete an Outside Employment Approval Request, per Fayetteville Policy and Procedure 450.1 PDF.
